Twenty-four nautical miles. It is one of the shortest hops in the catalogue, and as with almost every short route on this site, the first thing to say is when you do not need it. Quoted per aircraft on request.

Utila is closer to the mainland than to Roatán
This is the figure that reorders the decision, and almost nobody has it in mind.
La Ceiba–Utila is 22.3 nautical miles. Roatán–Utila is 24.0. Utila is closer to the mainland than to its neighbouring island.
And from La Ceiba there is a ferry: cheap, frequent and perfectly reasonable. So if your trip is going through the mainland anyway, the sensible routing is usually La Ceiba, not Roatán.
The specific aircraft is assigned on confirmation, based on passenger count, baggage and the destination field.
When flying from Roatán does earn its place
Three specific cases.
Chaining islands without returning to the mainland. If your itinerary is Roatán then Utila, dropping down to La Ceiba for the ferry and coming back up is a detour. Twenty-four direct nautical miles avoid it.
International departure day. If your flight home leaves Roatán on a thin margin, depending on a ferry that first takes you to the mainland is a real risk against an expensive ticket.
A rigid schedule. The ferry has its frequencies; you do not set them.
Outside those three, the ferry from La Ceiba solves the trip better and cheaper. We say so here so you trust us when we say the opposite.
Utila is not a high-capacity field
Útila handles regional and international traffic and is served daily by two airlines alongside numerous private and charter flights. It sits at 29 feet elevation.
It is an island aerodrome, not an international airport: the aircraft is decided by the field and your group, not by distance alone. The specific aircraft is assigned on confirmation, based on passenger count, baggage and the destination field.
A weather note about the origin
Roatán publishes no METAR of its own. The nearest station is Golosón, in La Ceiba, 73 kilometres away.
It is the same pattern we flagged at Tambor in Costa Rica: when the field does not report, the available weather information describes somewhere else. In an archipelago with microclimates, that distance matters. Several fields in the network operate in daylight and visual conditions, and close when the weather does not allow it.
The interval between diving and flying
Utila is a diving destination by definition, and there a constraint governs that is physiological rather than operational: there is a recommended interval between a last dive and flying.
The practical consequence: the day you transfer is not a full diving day. The interval between your last dive and the flight is the one your dive centre specifies.
