Puerto Cabezas is the most important port on Nicaragua's Caribbean coast, and it is not a natural harbor. It consists of a long pier reaching 400 meters (1,312 ft) straight out to sea. That single fact explains the entire coastline.
Why the pier is that long
There is no bay and no natural shelter along the whole coastline.
To reach usable depth, that structure had to be built offshore.
And what it means for navigation: this is an exposed coast without refuges, where sea state decides whether you go out at all.
The two names
Puerto Cabezas is the municipality's official name.
Bilwi is the Miskito name, meaning "snake leaf" — sources attribute the origin to different languages, Bawihka or Mayangna.
And English traders called it Bragman's Bluff.
Indigenous territory
The municipality and the entire region are indigenous lands.
Multiethnic population: Miskitu, Creole and Mestizo.
Local sources ask visitors to respect customs, languages and cultures, and add that showing genuine interest is generally very well received.
Ask before photographing people, hire community guides and spend locally.
The diving warning that applies region-wide
This is the most important thing on this page.
A commercial lobster diving industry operates in Miskito territory, and human rights organizations have characterized it as an occupational health emergency, after leaving thousands of divers with permanent decompression injuries.
Two practical consequences:
First: commercial lobster diving is not a tourist activity and should not be offered or contracted as one.
Second: a region where thousands have suffered decompression injuries indicates hyperbaric capacity that is limited relative to existing demand.
Which means: ask explicitly where the nearest hyperbaric chamber is and how long it takes to reach, before any dive. And confirm your insurance covers diving and medical evacuation.
Snorkeling is the sensible alternative here: same reef, no decompression risk, and no flight restriction afterward.
The Miskito Cays
A natural reserve and marine reserve off the North Caribbean Coast, described by sources as ideal for observing coral reefs, islands and marine biodiversity.
Protected status means its own regulations — verify what is permitted and where before contracting.
A historical note: a Portuguese slave ship wrecked in the Miskito Cays in 1642, an episode sources identify as one origin of the African component of the Miskito people.
The birds
The cays concentrate seabirds and coastal species, including migratory birds using the Central American Caribbean as a stopover.
Breeding colonies are especially sensitive to disturbance — an over-close approach can cause nest abandonment.
Keep the distance your guide sets, and ask what altitudes an operator maintains for any overflight.
The region after 2020
On 3 November 2020, Eta struck as a Category 4 with sustained winds over 240 km/h (150 mph).
Thirteen days later, Iota entered through the same North Caribbean Coast.
There were mass evacuations, including 682 minors from the Bismuna and Cabo Viejo communities.
Many communities remain in recovery. Approach with that awareness, and spend locally where possible.

From the air
The 400-meter pier reaching into the sea, which from above explains at a glance why it had to be built.
The Caribbean plain running inland without relief, drained by a dense river network.
And the Miskito Cays offshore, with reef visible under high sun.

Frequently asked questions
What does Bilwi mean?
"Snake leaf." Sources attribute the origin to different languages — Bawihka or Mayangna. It is the Miskito name for the city officially called Puerto Cabezas.
Why is the Puerto Cabezas pier so long?
Because it is not a natural harbor. There is no bay or shelter along the entire coastline, so reaching usable depth required 400 meters of structure built offshore.
Is it safe to dive in the Miskito Cays?
Ask explicitly where the nearest hyperbaric chamber is and how long it takes to reach. The region's commercial lobster diving industry has left thousands with permanent decompression injuries, which indicates limited hyperbaric capacity relative to demand.
What is the commercial lobster diving problem?
Human rights organizations have characterized it as an occupational health emergency. It is not a tourist activity and should not be contracted as one.
How long does it take from Managua?
One hour fifteen minutes by air for 368 km, against a bus of 10 to 12 hours. The city is 536 km from Managua by road.
How should visitors approach the region?
Local sources ask that customs, languages and cultures be respected, and note that showing genuine interest is very well received. Many communities are still recovering from the 2020 hurricanes.
